Adults transitioning from incarceration, older adults on a fixed income, working adults, and private-pay residents. Eligibility is confirmed during the application/referral process.

How fast is a referral or application processed?

Our goal is to respond to every referral within one business day.

What documentation do I need to refer someone?

Basic identifying information and any supporting documents you have on file. Full documentation can follow after initial contact.
